Every participant under 18 at the Pissouri 3rd Edition 2026 needs this form, completed and signed by a parent or legal guardian. Print it, fill it in and hand it in at bag pick-up (Friday 13 November, 18:00–20:00, Columbia Beach Resort) or at race-day check-in. Children without a signed form cannot start.

I am the parent or legal guardian of the participant and I am entitled to sign this form.
I have read the race manual and the Terms & Conditions and I accept them on behalf of the participant.
The participant can swim the chosen distance in open water and, to the best of my knowledge, is fit and healthy enough to take part.
I, or the adult named below, will stay at the venue during the participant’s race and will collect them afterwards.

In an emergency, if I cannot be reached, I agree that the event medical team may give the participant the first aid or medical treatment they consider necessary.

I consent to the organiser and the event medical team using this health information only to keep the participant safe during the event. It will be destroyed within 30 days after the event unless an incident occurs.

Race results are published with the participant’s name, category, time and place. You can choose otherwise:

The event is photographed and filmed for event coverage, our galleries and social media. We never publish a child’s contact details or date of birth with photos and we do not tag children.
Optional: I agree that photos or videos in which the participant is clearly identifiable may also be used in Cyprus Swim Series promotional material (for example adverts, posters and brochures). Leaving this unticked does not affect participation.
